APPLICATION FORM

You must be a resident of Spring Valley Village or Township and

not have been King or Queen in the past.

Contestants will be judged on appearance, personality, and answers given to judges.

King & Queen (grade 8-12):

1. Why is it so important to go to school and get a good education?

2.  What do you want to do after you graduate from high school?

3.  Give examples of things that you have done to help other people?

4.  What kind of club, organization, etc. are you involved in?

5.  If you could visit any place in the world, where would you go & why?

6.  If you had a million dollars how would you spend it?

Jr. King & Queen (grade 4-7):

1.  Where is your favorite place to go on vacation & why?

2.  What kind of clubs or organizations are you involved in?

3.  What have you done to help other people?

4.  Why do you like living in Spring Valley?

5.  Why is it important to get a good education?

Sr.  Prince and Princess (grade1-3) :

1.  If you could have any animal for a pet, what would it be and why?

2.  Where would you like to go on vacation & why?

3.  What do you want to be when you grow up?

5.  What is your favorite food made with potatoes?

Must be a resident of Spring Valley Village or Township. Also please dress in this year’s theme (International Year of the Potato)

Prince & Princess (kindergarten)

1.  What is your favorite food made out of potatoes?

2.  What is your favorite color? 

3.  What would you like to be for Halloween?

4.  What do you want to be when you grow up?

5.  If you could have any animal for a pet what would it be?
